Attending Without a Legal Guardian?
If your child will be coming to their appointment with a grandparent, other family member, or another trusted adult, please complete and sign the Vaccine Consent Form before the visit.
Having a signed consent form on file allows our care team to administer any recommended vaccines without needing to contact a parent or guardian during the appointment. This helps prevent delays, keeps the appointment on schedule, and ensures your child receives the care they need in a single visit.
